RN - Case Management

The Medical Management Department is seeking an RN Case Manager to provide a range of direct and indirect services for patients and their families, focusing on the identification and resolution of psychosocial problems. The RN Case Manager will collaborate with the patient, family, and interdisciplinary team to facilitate safe and timely discharge and treatment plans. Responsibilities Complete initial and concurrent insurance reviews. Complete PRI for patients requiring skilled nursing facility placement. Conduct psychosocial assessments using all available data, including interviews with patients and families. Ensure continuity of care through development and implementation of discharge/treatment plans. Function independently while utilizing supervision for problem identification. Maintain confidentiality according to hospital and Social Services policies. Participate in patient-focused conferences and internal/external conferences. Demonstrate leadership skills by utilizing psychosocial expertise for team building. Engage in departmental programs, including quality improvement and continuing education. Demonstrate knowledge of legal and ethical issues related to end-of-life care. Identify, report, and intervene for victims of violence and abuse. Advocate and negotiate with patients, families, and the community proactively. Required Experience / Certifications / Licensure RN License required. BSN preferred. Screener certification preferred. BLS Certification required.
