PRN Case Manager: Discharge & Care Transitions

Duke Health is seeking a Case Manager PRN for Duke Raleigh Hospital in Raleigh, North Carolina. The role focuses on care coordination, discharge planning, and coordination across the continuum of care to support safe transitions. You will work with interdisciplinary teams, patients and families, document appropriately, and ensure regulatory compliance.
