Job Description:

LPNs: Take Your Nursing Skills Into the Community

Full Time | Monday–Friday, 8 a.m.–5 p.m.
Palmer to Monson | $30–$38/hour, based on experience

Some of the most important nursing moments happen at home: noticing a change in a patient’s condition, helping a family feel confident about a care plan, or giving someone the support they need to stay safe and independent.

At Elara Caring , we’re looking for a Licensed Practical Nurse who wants to bring skilled, personal care to patients in the Palmer and Monson area. You’ll work alongside an RN Case Manager and a supportive clinical team while getting to know the people you care for.

What Your Day Could Include

• Providing nursing care in patients’ homes according to their plan of care
• Monitoring patients and sharing changes with the RN Case Manager
• Teaching patients and caregivers how to manage care at home
• Documenting visits, interventions, and medication updates
• Working with the care team to help patients avoid unnecessary hospital visits

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel throughout the Palmer and Monson territory
• Ability to sit, stand, bend, lift, and move intermittently, including lifting 50–100 lbs.
