Job Title: Physical Therapist - Home Health Location: Logansport, IN 46947
Duration: 6+ months with possibilities of extension
Shift: Monday-Friday, 8:00 AM - 4:30 PM Job Responsibilities:

Shift: Monday-Friday, 8:00 AM - 4:30 PM Job Responsibilities:
- Completes all patient evaluations and develops the PT plan of care within state specific guidelines
- Performs PT OASIS visits including Start of Care, Resumption of Care, Evaluation, and Discharges
- May supervise PTA care per company policy and state regulation
- Completes documentation within 24 hours of visit
- Attend and participate in branch meetings and case conference as specified by leadership
- Schedule flexibility is needed to ensure patient care needs are met
- Requires the ability to participate in a weekend rotation and potential availability on observed holidays
- Doctoral degree
- Current unencumbered Physical Therapy license for the State of Indiana
- Recent Home Care experience
- Proficiency with HomeCare HomeBase
- Understanding of Medicare Guidelines for Home Care
- Professional certification required
- CPR certification
- Driver's license and vehicle insurance with dependable vehicle
- Weekly Hour Guarantee: 36 hours
- Productivity Expectations: 30 points per week
- Coverage Areas: White, Pulaski, Fulton, Cass, Howard, Carroll, Miami, Tipton
- Mileage reimbursed between patient visits
- Occupational health screenings required
- Dress Code/Scrub Color: Navy Blue
