Job Title:  Home Health Aide 
Service Location: Effort, PA and Canadensis, PA
Rate: $14-$16/hr
Immediate Need:
Effort, PA - 9am to 3pm Monday through Friday until July 7th then transition hours to 2pm to 5pm throughout next school year
Canadensis, PA - 8 am to 6 pm Tuesday, Wednesday and Thursday

Responsibilities:

• Performs personal care/ household duties (e.g. dressing, toileting, feeding, light housekeeping) in home/school setting
• Maintain a clean and safe environment
• Provide emotional/behavioral support
• Follows plan of care
• Ability to interact as a member of a multidisciplinary team
• Reports any changes in the patient condition to agency/nurse case manager
