Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 Weeks Shift: Days Hours per Shift: 8 Experience: Minimum of 2 years in a Long Term Care setting License: Texas or compact license required Certifications: N/A Must-Have: 2 years PT experience in LTC setting, TX or compact license Description: The physical therapist will accurately assess and differentially diagnose patients. They will identify deficits in a patient’s ability to move and perform functional activities in their daily lives. The therapist will develop an individualized treatment plan with patient-centered goals. The aim is to reduce pain, restore function, and prevent disability. They will work with individuals to prevent the loss of mobility. Essential Functions and Responsibilities: Performs patient evaluations by obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. Establishes an individualized plan of care and selects treatment techniques that address specific impairments. Conducts ongoing reassessment of the patient and their response to treatment. Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment. Supervises and delegates to ancillary staff and students consistent with state laws and department policy. Completes legible, accurate, and clear documentation within the established timeframe. Communicates with the patient, family, caregivers, and members of the health care team. Assesses educational needs of the patient, family, and caregivers and provides education to meet those needs. Participates in professional growth and development through educational programs and current literature. Actively participates in quality improvement activities. Provides an environment conducive to safety for patients, visitors, and staff. Complies with appropriate approved safety standards. Functions within the scope of practice as defined by the state’s practice act. Performs other related duties as required.
