Hospice On Call Visiting Nurse

Hartford Healthcare is seeking a compassionate Hospice On Call Visiting Nurse to provide after-hours and weekend support to hospice patients in their homes and facilities. In this field-based role, you will manage pain and symptoms, perform urgent assessments, and offer education and emotional support to patients and families at end-of-life. You will collaborate closely with an interdisciplinary team, document care in the EMR, and ensure safe, high-quality, patient-centered care while working within a culture of teamwork, compassion, and professional growth.
