RN Home Health - Weekends

Role: Registered Nurse in Home Health, providing and coordinating nursing care in patients' homes. Key Responsibilities Develop and update patient care plans collaboratively with physicians and the healthcare team. Perform comprehensive nursing assessments, including OASIS and medication reconciliation. Provide high-quality clinical care, supervise and assign tasks to aides and LPNs/LVNs, ensuring compliance with regulations. Accurately document patient care, communicate effectively with physicians, patients, families, and staff. Participate in staff orientation, performance improvement, safety initiatives, and efforts to reduce hospitalizations. Maintain professionalism, punctuality, ongoing learning, and participate in on-call/weekend rotations. Adapt to diverse home environments regardless of client background. Qualifications & Skills Current RN licensure in the practicing state. Valid driver's license, vehicle insurance, and access to reliable transportation. Preferred: CPR certification (or ability to obtain within 90 days) and 1+ years of clinical experience.
