Essential Duties & Responsibilities Deliver High-Quality Primary Care Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities. Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness. Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent. Build Meaningful Patient Relationships Conduct regular facility rounds and develop strong relationships with residents, families, and care teams. Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations. Educate residents and caregivers on disease management, preventative care, and wellness strategies. Collaborate Across the Care Continuum Partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans. Serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions. Drive Clinical Excellence Support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions. Maintain accurate and timely documentation within the electronic medical record (EMR). Ensure compliance with applicable regulatory, clinical, and organizational standards. Qualifications Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field Masters Degree in Nursing or Physician Assistant Studies Strong leadership, interpersonal, and problem-solving skills preferred. Must possess excellent written and oral communication skills Must possess traits of punctuality, organizational skills, and ability to function within a team. This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
