Key Responsibilities\n- \n
- Conduct comprehensive assessments to identify and address the needs of patients, tailoring treatment plans accordingly. \n
- Maintain clear and effective communication with supervisors, family members, guardians, and multidisciplinary colleagues to update them on patient progress. \n
- Create and provide a structured schedule of appointments, home visits, and observations to monitor the advancement of treatment goals. \n
- Coordinate and facilitate in-home visits to perform thorough observations, including chart reviews and data analysis. \n
- Oversee the effective implementation of program plans and ensure they are delivered as intended to maximize patient benefits. \n
- Engage actively in Interdisciplinary Team Meetings, contributing insights and strategies that promote collaborative patient care. \n
- Document monthly and quarterly progress notes to track client development and adapt strategies as needed. \n
- Organize training sessions to ensure team members are skilled in program implementation and documentation requirements. \n
- Provide ongoing support through follow-up interviews and home visits, continuously assessing and responding to client needs. \n
- Maintain accurate and accessible records to ensure transparency and communication with stakeholders. \n
- Act as a liaison between our organization and the local mental health community, promoting collaboration and support. \n
- Assist in the development of training materials and procedures to enhance program effectiveness and staff proficiency. \n
As a Licensed Professional Counselor at Beyond Limits Health System, you will work with a diverse range of clients, including adults and youth facing various mental health challenges. Your expertise will be critical in providing high-quality care to individuals grappling with issues such as trauma, substance misuse, and relationship difficulties. Qualifications\n- \n
- Possession of a Master's or Doctoral Degree in Counseling, Psychology, or a related mental health field. \n
- Current and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona. \n
- A minimum of two years of experience in a counseling role, preferably within a clinical setting. \n
- A strong specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse is highly preferred. \n
- A solid understanding of the legal and ethical standards governing mental health practice. \n
- Exemplary interpersonal and communication skills, capable of building rapport with a wide range of clients. \n
- Ability to collaborate effectively with a multidisciplinary healthcare team. \n
- Proficiency in the use of Electronic Medical Record (EMR) software and related technologies. \n
