Develop and implement individualized therapy plans, monitor progress, and adjust goals as needed. Deliver direct speech-language pathology services, document treatment notes, and maintain accurate records in the charting system. Collaborate with an interdisciplinary team (nursing, rehabilitation, social work, and medical staff) to optimize patient outcomes. Screen for and manage swallowing disorders; provide dysphagia strategies and educate residents, families, and caregivers. Support discharge planning and transitions of care, communicating changes in status and contributing to care conferences.
