Responsibilities & Requirements Physical Therapist , Dayton Osteopathic Hospital, Huber Heights, OH. Evaluate & treat patients in accordance with state & national practice acts. In cooperation with referral source, provide therapy evaluation to determine extent of symptoms & or functional limitations relevant to diagnosis. Develop treatment plan that is targeted & adjusted to decrease or eliminate symptoms & functional limitations. Direct patient care, day‑to‑day patient scheduling, supervision of Physical Therapist Assts & students, consultation with physicians & other team members, documentation in electronic medical record, QA & performance improvement (QAPI), clinical education, guest relations & billing for patient services rendered & other duties as assigned. Minimum Requirements : Bachelor’s in Physical Therapy from an accredited program, active State of Ohio Physical Therapist License & CPR certified.
