JOB DESCRIPTION

POSITION TITLE: Older Adults Housing Case Manager

CLASSIFICATION STATUS: Exempt

POSITION DESCRIPTION: The Older Adult Housing Case Manager resides within the Housing & Health Navigation department and is part of the Centralized Intake team. They will employ an integrated housing and care management service delivery model to improve housing and health outcomes for people living with HIV, with a specialized focus on adults age 55+.

The OAHCM provides intensive case management, including permanent housing placement (PHP) and short-term rental, mortgage, and utility assistance (STRMU), to adults ages 55+ and any other eligible participant in need of these services. Working with Chicago House staff, external medical providers, landlords, and community-based agencies, the OAHCM addresses barriers related to social determinants of health and supports participants in obtaining and maintaining stable housing.
