PRIMARY FUNCTION: The Community Based Services Coordinator is responsible for supervising the direct support providers (DSP) in carrying out the individualized day program plans such as ISSPs, IBSSPs, Protocols and Rights Modifications for all KAAUM PRS within Day Program, CES, CHRP, Supported Employment, and SLS services. The day program coordinator will work with the DSPs to ensure that they receive individualized plans for the community setting that supports the personal needs of their plans. The day program Coordinator is also responsible for managing the PRS files, reaching out to the CCB and other PASA agencies to ensure that the files are most up to date.

MINIMUM QUALIFICATIONS

Ø pass a background and CAPS check.

Ø have a ‘clean’ DMV record, in accordance with company policy and state rules and regulations.

Ø be 21 years of age.

Ø possess a valid US driver’s license.

Ø have a current CPR & First Aid certification. *

Ø have a current Crisis Prevention Training certification (i.e., Safety Cares, Mandt, CPI). *

Ø supply proof of auto insurance including an insurance declaration page stating coverage.

Ø be willing to adapt to schedule changes to meet PRS needs.

Ø physical requirements essential for performing the duties of this position

 

*Courses to obtain or renew this certification may be provided (or compensated for) by KAAUM Enterprises.

 

Ø Preferred qualifications:

o Bachelors preferred – in the areas of social work, rehabilitation counseling, psychology, nursing or other related fields.

o 2+ Years of social work experience

o Knowledge and understanding of Intellectual Developmental Disabilities

 

POSITION DUTIES AND RESPONSIBILITIES

Ø PERSON RECIEVEING SERVICES (PRS SUPPORT)

o Act as a liaison, advocate and coordinate for PRS involvement in self-advocacy, vocational programming, transportation services, integrated community activities, recreation, and learning independent living skills.

o Foster open and supportive communication with PRS families, Host Home Providers, and other community organizations and agencies. (As needed)

o Implement and follow up on all support service recommendations

o Ensuring that the PRS are transported to and participate in all activities with full involvement from the DSPs who are responsible for each PRS in their care.

o The Community Based Services coordinator is expected to be an active participant in all calendared company events.

o If there is a staffing shortage the Community Based Services coordinator will step in and provide services to the PRS following their same schedule.

 

Ø SUPERVISION/TRAINING

o The Community Based Services coordinator answers to the Community Based Services managers as their direct supervisor.

o The program coordinator is responsible for informing the Community Based Services manager about pertinent issues.

o Monitor DSPs performance by conducting day program monitoring.

o Arrange and schedule all training for all direct care providers.

o Supervise the DSPs training and development by conducting training reports.

o Present and implement all agency policies and procedures.

o Participate in the interviewing of direct care providers.

o Review DPS log notes to ensure they meet state standards.

 

 

Ø MEETINGS/FUNTIONS

o Attend all staff meetings and fill in on facilitating meetings if the Community Based Services manager is absent.

o Coordinate/attend as needed all consumer related meetings such as IDTs, CSRs, and SOR meetings and ensure all follow-up is completed.

o Participate in special projects and committees.

 

Ø REPORTS/PAPERWORK

o Ensure completion/submission of all required daily, monthly, quarterly, and annual reports and assessments.

o Routinely conduct file audits to ensure files are current and up to date.

o Processing and submitting IRs/GERs to all applicable parties as well as being responsible for all follow-up.

o Ensure monthly calendars are completed for all DSPs, as well as conducting their quarterly fire and safety drills.

 

Ø MAINTENANCE

o Oversee DSPs annual vehicle inspections completion- to include cleanliness, safety issues, emergency situations.

o Facilitating all required maintenance for company vehicles

o Ensure accurate documentation for the vans is completed

o The Community Based Services coordinator agrees to cooperate with Agency and CDPHE/HCPF staff during any inspection or investigation and or state audit.

o Perform additional duties as required by the nature of the position or as requested by the day program manager and or senior administration personnel.

 

PHYSICAL REQUIREMENTS

Ø Standing for extended periods.

Ø Ability to walk long distances and hike over varied terrain.

Ø Driving for sustained durations.

Ø Crouching or kneeling, particularly for tasks requiring close attention or detailed work.

Ø Reaching, bending, and twisting as needed for various tasks.

Ø Ability to lift and carry objects weighing up to 25 lbs. or more as required.
