Responsibilities
- Conduct comprehensive occupational therapy evaluations for adult residents with acute care needs.
- Develop and implement individualized treatment plans aimed at improving activities of daily living, mobility, and cognitive function as appropriate for the setting.
- Reassess functional goals regularly, document progress, and modify plans to support successful discharge or transition within the care continuum.
- Collaborate with an interdisciplinary team (physical therapy, speech-language pathology, nursing, social work, and therapy aides) to coordinate care and optimize outcomes.
- Provide patient and family education on self-management, safety, and adaptive strategies; ensure appropriate home or community plan is arranged.
- Maintain accurate, timely documentation in patient records in compliance with regulatory and facility requirements.

Requirements
- Active Occupational Therapist license in Massachusetts or eligibility to obtain licensure.
- Graduation from an accredited OT program (degree appropriate to licensure).
- Experience or demonstrated ability to work in acute care or skilled nursing settings; strong communication and teamwork skills.
- Ability to work daytime, 5x8 shift patterns and participate in travel assignments as required.
- Demonstrated clinical reasoning, documentation accuracy, and commitment to patient safety.
- Comfortable functioning within a multidisciplinary team and adapting plans to resident goals.
