Lynda’s Home Health Care is seeking a compassionate and dependable Registered Nurse (RN) to join our growing home health team in Santa Monica and surrounding areas. The RN will provide skilled nursing services to patients in their homes while coordinating care with physicians, therapists, caregivers, and family members to ensure high-quality patient outcomes. Responsibilities Conduct patient assessments and develop individualized care plans Provide skilled nursing care including wound care, medication management, injections, catheter care, and monitoring of chronic conditions Educate patients and families regarding diagnoses, medications, treatments, and disease management Coordinate care with physicians, physical therapists, occupational therapists, and other healthcare professionals Maintain accurate and timely clinical documentation in compliance with state and federal regulations Monitor patient progress and report changes in condition to the physician and care team Ensure compliance with Medicare, HIPAA, and agency policies and procedures Participate in case conferences and interdisciplinary team meetings Qualifications Active California Registered Nurse (RN) license Minimum of 1 year nursing experience preferred (home health experience is a plus) Current CPR/BLS certification Strong clinical assessment and communication skills Ability to work independently and manage time effectively Reliable transportation and valid driver’s license Knowledge of electronic medical records/documentation systems preferred Benefits Flexible scheduling Competitive pay Supportive team environment Pay: $45.00 - $70.00 per hour Work Location: In person #J-18808-Ljbffr Rose Senior Living - Avon
