- Expected start date (First day should always be reserved for orientation tasks): ASAP Ideally by Mid-October

- FTE needed: 1.0 (want to fill with 1 locum provider but open to 2 if they work opposite of each other 1 week on 1 week off or 2 weeks on 2 weeks off)

- Required clinical activity in the last 24 months (case logs): Applicant must provide documentation (case list) of provision of clinical services (100 cases) representative of the scope and complexity of the privileges requested during the previous 24 months.
- Will provider work solo? If not, other providers specialties working with them: They will work with our multidisciplinary team- EP, Interventionalist, APC's RN's, MA' Cardiac Sonographers, Device Tech's
- Licensed Candidate: Yes/No: Washington license is preferred, but we are open to IMLC with LOQ in hand.
- Required certifications (Must be from American Heart Association or American Red Cross as applicable): ACLS
- EMR: EPIC
