FTE needed: 1.0 (want to fill with 1 locum provider but open to 2 if they work opposite of each other 1 week on 1 week off or 2 weeks on 2 weeks off)

Required clinical activity in the last 24 months (case logs): Applicant must provide documentation (case list) of provision of clinical services (100 cases) representative of the scope and complexity of the privileges requested during the previous 24 months.
Will provider work solo? If not, other providers specialties working with them: They will work with our multidisciplinary team- EP, Interventionalist, APC's RN's, MA' Cardiac Sonographers, Device Tech's
Licensed Candidate: Yes/No: Washington license is preferred, but we are open to IMLC with LOQ in hand.
Required certifications (Must be from American Heart Association or American Red Cross as applicable): ACLS
EMR: EPIC
