Job Description
Physical Therapist

We are seeking an experienced Physical Therapist to provide high-quality care in a home health setting. The ideal candidate will possess a strong background in home care and home health practices.

Responsibilities

Conduct evaluations and assessments of patients' physical conditions.
Develop personalized treatment plans and administer physical therapy interventions.
Perform start of care (SOC), recertifications (ROC), and evaluations as required.
Deliver daily patient visits and ensure continuity of care.

Qualifications

Current and valid Physical Therapy license.
BLS certification.
Experience in home care and home health is required.
Preferred: Previous experience rather than new graduates.

Patient Demographics

Common diagnoses and conditions include congestive heart failure (CHF), orthopedic issues, and pneumonia.

Work Environment

The Physical Therapist will work in patients' homes and may cover various locations within the area.

Shifts and Scheduling

5-8 hour shifts are available.
Rotating weekend shifts from 8 AM to 5 PM are required.
Participation in holiday rotation is expected.

Orientation and Support

New hires will undergo a comprehensive 2-week orientation with the support of a work coach for guidance during patient visits. Continuous support from the clinical manager or educator is available 24/7 for emergencies and from 8 AM to 5 PM for non-urgent queries.

Attire

Scrubs and business casual attire are acceptable.
