Responsibilities include conducting home visits, completing intake and assessment documents, collaborating with medical providers and community partners, monitoring and following up on cases, maintaining accurate documentation, conducting presentations, attending community events, and performing duties as assigned. The role also supports incident command structure in public health emergencies and complies with HHS, DSHS, and Texas Department of Public Safety policies. Essential Job Functions (40%) Provide complex and specialized social work consultation and comprehensive case management services, including intake, family needs assessment, service plan, and follow-up; assist families with eligibility determination; provide information and referral services; maintain documentation and use database and software tools. (20%) Conduct comprehensive assessments to determine eligibility and need for PCS/CFC, document identified needs, forward documentation for authorization, conduct reassessments, and enter encounter information into an online database. (10%) Provide consultation, presentations, and training to service providers and contractors; perform public relations; assist in training new staff, volunteers, and social work interns. (20%) Actively participate in community organization functions and collaborate with community entities; serve as representative for DSHS on CRCGs and advocacy groups; participate in disaster mental health services; maintain community work plan and resource directory. (5%) Attend trainings, conferences, staff meetings, and performance evaluation processes; provide backup and support to team members. (5%) Perform additional duties as assigned, including disaster response or recovery tasks and continuity of operations activities. Knowledge, Skills, Abilities Knowledge of Case Management principles and practices. Knowledge of children’s programs such as Medicaid, CSHCN, Case Management for Children and Pregnant Women, CPS/Foster Care, SSI, NBS, NBHS, Lead, Early Childhood Intervention (ECI), and CHIP. Knowledge of quality assurance and technical assistance policies and procedures; knowledge of community resources at local, state, and national levels; knowledge of cultural and socioeconomic issues related to health and disabilities. Knowledge of evolving issues within the greater social work realm. Skill in using computer software programs including MS Word, PowerPoint, Excel, Access, and Outlook. Skill in compiling monthly, travel, and time activity reports. Skill in conducting in-home family needs assessments and developing service plans. Skill in providing presentations and trainings. Skill in conflict resolution, mediation, and problem solving. Ability to work effectively as a team member in an interdisciplinary setting. Ability to interpret and apply program policies and procedures. Ability to provide professional leadership. Ability to maintain confidentiality. Ability to plan work in an orderly manner to manage caseloads and meet deadlines. Ability to provide technical assistance to professionals. Ability to speak and write effectively and address audiences professionally. Ability to provide own transportation for work duties. Ability to work under minimum supervision with considerable latitude for initiative and independent judgment. Ability to continue key educational activities on current social work topics. Ability to follow DSHS Policies and Procedures and adhere to DSHS Standards of Conduct and Work Rules. Registrations, Licensure Requirements or Certifications Current, unrestricted license to practice social work in the State of Texas. Initial Screening Criteria Bachelor’s or graduate degree from a Council on Social Work Education accredited college or university. Additional Information Master's Degree preferred. Bilingual (Spanish) preferred.
