Key Responsibilities Home Visits: Conduct regular, personalized visits to evaluate and support patients' needs. Comprehensive Assessments: Thoroughly assess physical, psycho-social, and environmental factors in the home. Patient & Family Education: Empower families with clear guidance and resources to enhance quality of life. Collaborative Care: Partner with multidisciplinary healthcare teams to ensure seamless care delivery. Documentation & Supervision: Maintain timely and accurate visit documentation, and provide guidance to home care aides. Qualifications Licensure: Active Registered Nurse (RN) license in good standing (Virginia state). Education: Graduate of an accredited nursing program (Associate, or Bachelor’s Degree). Certifications: Current CPR / BLS certification. Experience: 1 year of RN home health/home care experience preferred. Requirements: Valid driver’s license, reliable transportation, and active auto insurance.
