Key Responsibilities: Conduct thorough assessments and develop customized physical therapy treatment plans. Apply therapeutic interventions to enhance mobility, strength, posture, gait, and balance. Fit and instruct residents on the use of adaptive equipment such as orthotics and prosthetics. Select appropriate physical modalities based on residents' diagnoses and physical capabilities. Collaborate with a multidisciplinary team to ensure cohesive, resident-centered care. Monitor progress, adjust therapy plans as needed, and educate residents and caregivers. Contribute to care planning by participating in interdisciplinary meetings. Supervise and train support staff, ensuring high standards of therapy delivery. Lead in-service education sessions for nursing and medical team members. Oversee the ordering and maintenance of therapy supplies and equipment. Qualifications: Active, unencumbered New Jersey Physical Therapist license. Strong communication, organization, and teamwork skills. Passion for working with seniors and enhancing quality of life through rehabilitative care.
