Nurse Practitioner

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities Provide effective, equitable primary care to adult Medicare patients, focusing on keeping them healthy and out of the hospital. Conduct office visits for routine and acute issues, including annual wellness visits and health assessments. Collaborate with a multidisciplinary care team and coordinate with specialists and testing facilities. Lead population health initiatives, ensuring preventive screenings and health management. Assist with phone triage, patient education, outreach, and conduct home visits as needed. Participate in promotional activities and other assigned duties. Qualifications & Skills Master’s or Doctorate in Nursing or NP certification, with active state license and DEA registration. Certification in Family, Adult-Gerontology, or Gerontological Nurse Practice. Experience in primary care, geriatrics, or family medicine is preferred. Knowledge of HCC documentation, ICD-10 coding, and health risk assessments is a plus. Strong teamwork, communication skills, and a passion for population health. Bilingual abilities are advantageous.
