Job Description Evaluates, plans, treats, and implements care for patients in accordance with the American Physical Therapy Association standards and Essentia Health policies and procedures. Key Responsibilities Provide therapy evaluation, develop individualized treatment plans, and implement evidence‑based interventions. Provide patient/caregiver education and discharge planning for a full patient caseload. Complete all ancillary and administrative requirements for a full caseload, including documentation, billing, scheduling management and other duties. Demonstrate appropriate communication, professionalism and supervise support staff (licensed assistants, aides, volunteers). Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes. Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care. Comply with organizational policies, procedures, code of conduct, professional organization core values, ethics, and scope of practice. Be flexible to work in other departments when patient need is highest and home department schedule allows. May serve as a clinical instructor, participate in department or discipline committees, and complete credentialing or certification benefitting patient care. In inpatient acute care: coordinate and deliver durable medical equipment and collaborate with interdisciplinary team to support smooth transitions of care. Qualifications Must hold a current state license to perform physical therapy services. Bachelor’s, Master’s, or Doctorate degree from an accredited physical therapy program. Demonstrated flexibility, adaptability, and ability to thrive in a fast‑paced environment. Will provide a full caseload of patients—may require weekend rotation once every 4 weeks (minimum). Shift & Scheduling Shift Rotation: Day Rotation (United States), shift start and end times: (not specified). Weekend rotation once every 4 weeks. Holidays: No. No call obligation.
