Description: The RN utilizes an interpersonal process that focuses on the total individual and his/her role in their family, culture, and society. The RN utilizes the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The RN fulfills a resident advocacy role by ensuring the resident and/or their representative has adequate information to make health decisions. The RN coordinates total care and works with the resident to achieve or maintain their highest practicable level of function. The RN is guided by substantial specialized knowledge of nursing based on physiological and psychological sciences. The RN ensures that the quality of resident care meets legal and professional standards. The RN also acts as a role model to other members of the healthcare team. This position requires full understanding and active participation in fulfilling the Mission, Vision, and Values of the organization. It is expected that the associate will demonstrate behaviors consistent with the core values. The associate shall support the organization's goals and direction of the quality improvement process. The RN completes holistic, age-specific admission history, physical assessments, and nursing plans of care on all assigned residents. The RN analyzes and incorporates clinical data to formulate an individualized plan of care. The RN identifies and documents residents at risk for nutritional and functional status. The RN identifies and reports signs/symptoms of neglect or abuse. The RN sets priorities for and reviews daily plans of care in collaboration with residents, families, and other healthcare disciplines. Delegation to team members is based on resident needs, continuity of care, staff, and knowledge. The RN communicates with physicians to ensure coordination of medical and nursing plans of care. The RN demonstrates competence in performing technical skills. The RN proactively recognizes and demonstrates ability to manage urgent/emergent resident care situations. The RN demonstrates knowledge and technical competence in the preparation and administration of medications and IVs per scope of practice and organization policy. The RN documents nursing interventions, residents' responses to care, and resident outcomes. The RN evaluates data to revise the plan of care to meet the changing needs of the resident. The RN evaluates responses to interventions against expected norms for age and development. The RN identifies and documents resident/family learning needs and provides education based on assessment in collaboration with residents, families, and interdisciplinary team members. The RN identifies and documents discharge planning needs. The RN coordinates or delegates discharge activities and completes or validates transfer/discharge documents. The RN provides instructions related to medications, activity, wound care, and follow-up appointments. The RN documents resident/family responses to education. The RN maintains working knowledge of regulations to ensure documentation supports and reflects compliance. The RN maintains knowledge of criteria to ensure documentation supports the need for care and reflects resident status. The RN incorporates resident needs and preferences into organized, efficient delivery of care by a team assigned for daily cares. The RN supervises, coaches, counsels, and evaluates the performance of assigned staff. Required skills include the ability to read and write at a level to fulfill documentation requirements. Physical demands involve lifting over 100 lbs, with frequent lifting/carrying up to 50 lbs.
