Hospice RN Case Manager — 1:1 Home Care & On-Call

St. Croix Hospice in Minnetonka, MN is seeking a Registered Nurse Case Manager to provide compassionate care and holistic support to patients and their families in their homes. This full-time role requires an RN with recent acute care experience and the ability to develop individualized care plans. The Registered Nurse will partner with an interdisciplinary team, guide end-of-life conversations, and ensure comfort with professionalism and empathy.
