Physical Therapist

Coverage Area: North Snohomish County (North Marysville, Granite Falls, Arlington, Camano Island, Stanwood) Schedule: Monday–Friday, 8:00 AM – 4:30 PM Hours: 40 hours/week (8 hours/day) Productivity Expectation: 30 points Travel: Required within service area (Mileage reimbursed between patient visits) About the Role We are seeking a compassionate and skilled Physical Therapist (PT) to provide in-home care services to patients across the North Snohomish County region. In this role, you will evaluate, treat, and support patients in improving mobility, reducing pain, and achieving functional independence. You will work collaboratively with physicians and interdisciplinary care teams to develop individualized care plans and ensure high-quality, patient-centered outcomes in a home health setting. What You’ll Do Patient Care & Treatment Evaluate patient functional status and physical therapy needs, including all body systems required for CMS documentation Develop and implement individualized Plan of Care in collaboration with physicians Provide therapeutic treatments to relieve pain, restore function, and maintain mobility using evidence-based practices Monitor patient progress and adjust treatment plans as needed Documentation & Compliance Complete evaluations and establish care plans within state-specific guidelines Submit progress notes for each visit in compliance with company policy Document patient status, treatment outcomes, and care frequency within 24 hours of evaluation Prepare discharge summaries and periodic reports (at least every 60 days) Communicate patient progress and changes in condition to physicians and nurse supervisors Educate patients, families, and caregivers on treatment plans and functional goals Participate in interdisciplinary team meetings, staff conferences, and committees Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy Oversee home health aides when applicable Provide in-service education as requested Operational Responsibilities Travel throughout the assigned service area to provide care Maintain schedule flexibility to meet patient care needs Support agency-wide quality and performance standards Minimum Qualifications Current Physical Therapy license in Washington (or compact license eligible to practice in WA) Current CPR certification Valid driver’s license, vehicle insurance, and access to a reliable vehicle Experience in physical therapy practice (home health experience preferred) Preferred Qualifications Experience in home health care settings Familiarity with HomeCare HomeBase (HCHB) or similar EMR systems Strong knowledge of CMS documentation and regulatory standards Skills & Attributes Strong clinical assessment and decision-making skills Excellent communication and patient education abilities Ability to work independently in a home-based environment Strong organizational and time management skills Commitment to high-quality, patient-centered care Home health setting (patient homes) Travel required within designated service area Mileage reimbursement provided between patient visits Scrubs required (solid color, no prints) What Success Looks Like High-quality patient outcomes and improved functional independence Timely and accurate clinical documentation Strong collaboration with interdisciplinary teams Consistent achievement of productivity goals Equal Opportunity Employer Statement We are an equal opportunity employer committed to creating a diverse and inclusive workplace.
