Family Practice/Primary Care Nurse Practitioner

This Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) role is based in Astoria, Oregon , at a community health center recognized for its strong team-based care model, deep community roots, and commitment to access for underserved populations. The clinic operates within a patient-centered medical home structure and offers robust clinical support, including experienced MAs, care coordination, integrated services, and built-in language support.
