Supportive Services for Veteran Families, Summit County

Full-Time, 40 Hours/Week

$42,000/Year

SCHEDULE: Determined on a case by case basis

SUMMARY: Community-based case manager for homeless and at-risk veterans experiencing housing crises. 

SUPERVISOR: Program Manager

DUTIES AND RESPONSIBILITIES:

•Travels to community locations where homeless and at-risk veterans congregate to ensure effective local outreach.

•Assesses needs, risks, and obstacles applicable to household stability and long-term permanent housing.

•Assures the provision of financial assistance for homeless prevention or rapid re-housing services as needed and requested.

•Assures that participant housing is habitable according to established organizational standards.

•Coordinates and brokers community resources that meet individual needs.

•Assists participants in developing realistic and achievable housing plans that focus on housing stability and include measurable action steps and timetables. 

•Maintains regular contact with the household including childcare assessments, crisis intervention, referrals, and ongoing life skills development.

•Assesses progress toward goals, updating and modifying individual service plans as needed.

•Documents and records all significant interventions, diagnostic action steps, progress reports, utilization of referral services.

•Collects and provides required data for the HMIS database. 

•Actively participates in regular SSVF Coordinating Team meetings for the purpose of program consistency, monitoring and effectiveness.

•Provides crises intervention when necessary.

•Applies Housing First and Motivational Interviewing best practice models in service delivery approach.

•Participates in community-wide events promoting services for veterans.

•Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

•Other duties as assigned.

MINIMUM QUALIFICATIONS:

•College-level coursework in human services or related field preferred.

•Experience working with veterans preferred.

•Experience with the issues of homelessness, domestic violence, chemical dependency, mental health preferred.  

•Demonstrated organizational, interpersonal, and communication skills

•Willingness to learn and apply best practice models as appropriate

•Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability if applicable; reliable transportation

•Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation). 

LICENSES OR CERTIFICATIONS REQUIRED:

•Unrestricted State of Ohio driver’s license

While performing the duties of this job, the employee is frequently required to use close vision (clear vision at 20 inches or less). The employee will occasionally be working near moving mechanical parts, be exposed to outdoor weather conditions and exposed to a risk of electrical shock while running the copier, printer, other standard equipment and answering the telephone. The noise level in the work environment is low to moderate. Stress level can be high at times.
