Our extensive array of community-based services are fully integrated, bilingual and culturally competent.The CNA is a member of Nursing Team who is responsible for performing assignments as delegated by a licensed Nurse. All assignments are performed under the supervision of an RN or LPN, in compliance with facility policy/procedure, within the scope of the CNA's responsibility to provide safe care.Key Essential FunctionsAssists in meal/nutrition related activities including, but not limited to: Sitting residents in dining roomDistributing meal trays to residents in their room and assisting residents with their trays in the dining roomFeeds, and/or helps resident to prepare tray and feed themselves, reports, food errors and/or need to obtain substitute meal itemsOffers and provides fluids (e.g. water, juices, tea, coffee, etc.)Offers and provides snacks/nourishmentsReports any changes in appetite, ability to consume food/fluidWeighs residents, records weight, and reports significant changes to licensed nurseAssists resident in toileting needs, including but not limited to: Toxicology Screening, collects urine from individual patientsAssists patients with changing incontinent briefsReporting any changes in bowel/bladder patterns or habitsAssists resident with daily living activities which include but are not limited to: DressingHair combing/brushingTakes an interest in the residents' involvement with activities and assists with recreational activities.Assists with admission and discharge activities.Accounts for linen and helps keep linen area neat and orderly.Reports to unit nurse when unit supplies need to be replenishedParticipates in maintaining the Environment of Care by: Observation or notification of possible possession of contraband or items that do not belong to him/herSearches of room area in order to prevent patients from concealing dangerous contraband materialsReporting safety hazards and removing overt hazardsReporting any resident incidents, accidents, fallsResponding to fire and/or disaster alarmsMaintaining the residents room area by bed making, tidying drawers and closets, helping residents to place personal items such as photographs, etc. in the place they would likeEnsuring that beds are left in a safe, low position after resident care or bed-makingParticipates in the management of clothing for residents which includes but is not limited to: Conducts admission searches.Conducts searches of patients and their personal belongings, in accordance with the agency search policy.Reporting clothing needsReporting need for clothing repairArranging for and/or assisting with residents' use of the washer and dryer on the unitPerforms sanitizing and safety precaution procedures required for on unit washer and dryerParticipates in the surveillance, prevention and control of infection by: Adhering to Infection Control Policies and ProceduresObtaining specimens and give to lab technician as directedMeasuring and recording vital signs (BP, T, P, R,O2sat) and reporting any abnormalitiesParticipates in the Continuum of Care and the Education of residents by: Assisting with/facilitating other exercise programs on the unit as directedInteracting with residents and communicating their expressed concernsApplying/adjusting for comfort supplemental gas delivery apparatus (oxygen face mask or nasal prongs)Attending scheduled meetings and In-service Education programsTransport/accompany detox patients as requested by nurses managers: Light lifting required when appropriateMonitors patients for any change in condition: Reports changes in skin condition to licensed nurseEnsures proper positioning of resident to prevent/minimize skin breakdownAdheres to incontinence care programs (i.e. frequent brief changing, toileting, perinea cleansing, etc.)RequirementsHS Diploma/GEDValid and unrestricted NYS Certification as a Nursing Aide.Current Basic Life Support certification.Must obtain Mandated reporter (2 hours) within thirty days from hire date.Obtain within six months of hire: Supporting Recovery with Medications Addictions Treatment (MAT)CASAC Canon of Ethics (6 hours)Confidentiality related to 42CFR (3 Hours)Bilingual (English/Spanish), preferred.Ability to work on a strong team of professionals in a culturally diverse environment.Ability to multitask and to work and function under pressure.Why Join Us?Acacia Network provides a comprehensive and competitive benefits package to our employees.
