Nurse Practitioner (NP) - Hybrid Role

Job Summary:

Carease Health - NC is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality preventative care to residents in skilled nursing facilities in Wake Forest, NC. As a hybrid practitioner, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home

• Build and maintain strong relationships with facilities, maintaining a patient-centered approach

• Collaborate with a dedicated scribe team for efficient documentation

• Travel to facilities in Henderson, NC only 2 days per month

• Participate in quality improvement initiatives to minimize hospitalizations and elevate patient care standards

Requirements:

• Active NP or PA license in the state of North Carolina

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Ability to work 11 hours per week total between two nursing homes in Henderson
