Licensed Practical Nurse (LPN)

General Purpose Under the supervision of a staff Registered Nurse or the RN supervisor, the LPN provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and agency clinical policies. Education: Graduate of an accredited Practical Nursing Program. Licensing: Licensed to practice as a Licensed Practical Nurse with active license in the state of Maryland of compact states. Maintenance of current Cardio Pulmonary Resuscitation (CPR) for HealthCare Workers or Basic Cardiac Life Support (BCLS) certification for HealthCare workers. Educated (Agency will train) on and compliant with HIPAA regulations; maintains strict confidentiality of client information. OTHER SKILLS AND ABILITIES: Nursing skills, observation skills, communication skills, and knowledge of home health care and government regulations. Good physical and mental health. Caring attitude, empathy, tact, and patience. Essential Functions Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the RN supervisor and/or a Registered Nurse designee. Ensures quality and safe delivery of care to recipient. Implements plan of care formulated by physician and Registered Nurse. Provides accurate and timely documentation consistent with plan of care. Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care. Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards. Participates in coordination of care to recipient with Registered Nurse and clinical management team. Assists patient in learning appropriate self-care activities Uses equipment and supplies effectively and efficiently. Documents and reports activity as required.
