Social Worker

Pay Range $29.55 - $44.32 per hour. Pay determined by experience, skills, training, certifications and education. Position Details Title: Social Worker Department: Care Management Core Function: Behavioral Health/Social Work Schedule: 40 hours/week, every 5th weekend Facility: St. Vincent's Hospital Location: Green Bay, WI Compensation: aligns with experience Responsibilities Coordinate and provide social services for patients of all ages in the hospital setting. Assist patients and families in navigating the medical system, assess and monitor mental and emotional health, deliver short‑term counseling and therapy, and communicate patient needs to the multidisciplinary team. Education Qualifications Master's Degree in Social Work (required) Bachelor's Degree in Social Work plus five years of clinical experience may be considered in lieu of a master's degree. Experience Qualifications Prior experience in social work or related social service clinical experience in a healthcare environment (preferred) Two years of home care, hospice, discharge planning or related experience (preferred) Licenses, Certifications, and Other Requirements Licensed as a Certified Social Worker (CSW) in Wisconsin or a Licensed Social Worker (LSW) in Illinois (required) Preferred Wisconsin credentials: Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), Licensed Clinical Social Worker (LCSW) Preferred Illinois credential: Licensed Clinical Social Worker (LCSW) Basic Life Support (BLS) certification (required) Valid driver's license, automobile insurance and own transportation required if travel is involved Benefits HSHS offers comprehensive benefits including medical, prescription, dental, and vision coverage; paid time off; retirement benefits; education assistance; adoption assistance; wellness program; life and disability coverage; flexible spending accounts; employee assistance program; and more.
