Staff Nurse CN II - Outpatient-CFP

Staff Nurse CN II - Outpatient-CFP, 2026-1292 UConn Health is searching for an experienced Staff Nurse CNII with Complex Family Planning experience. The CFP nurse will provide extensive patient navigation, care coordination, address social determinants of health needs, and provide emotional and social support. This position will work in UConn Medical Group's General OB office and may travel to offsite locations. Schedule 40 hours per week, Monday-Friday, 8‑hour shifts between 7:30 a.m. and 6:30 p.m. (60‑minute unpaid meal break). Responsibilities Perform individualized nursing assessments for each assigned patient. Organize and coordinate patient care. Collaborate with the health care team to implement patient care plans. Develop and update care plans using established standards of care, including physician‑prescribed therapies and common nursing measures. Communicate and educate patients and family members for a clear understanding of the care provided. Complete required documentation in a timely manner according to policies and standards. Contribute to the quality of nursing care through performance improvement. Participate in staff and patient care conferences. Assist in clinical education, orientation, and mentoring of other staff. Perform related duties as required. Minimum Qualifications Knowledge of principles and practices of nursing. Knowledge of relevant health center policies and procedures and State regulations. Knowledge of clinical standards of care, documentation and patient record policies and standards. Understanding of computer‑based applications. Considerable interpersonal and communication skills. Ability to work as an effective team member. Ability to establish priorities and manage effectively. Experience and Training Graduation from an accredited nursing program. One (1) year professional nursing experience. Current registered nurse license to practice nursing in the State of Connecticut. Preferred Qualifications Experience in Family Planning and Reproductive Health. Women’s Health/OB experience. BSN or active enrollment in a BSN program. Working Conditions May be required to lift and restrain patients; may be exposed to communicable/infectious diseases and to risk of injury from assaultive/abusive clients; may be exposed to disagreeable conditions. Salary Minimum equivalent annual salary: $84,656.00 plus additional compensation based on experience.
