Licensed MI/CD Psychotherapist

Job Summary Come join our Princeton team, located in the Northland Hospital! Reviews assessments at time of admission, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member. Responsibilities Demonstrates clinical competence in the treatment planning process. Facilitates treatment planning and oversees management of each case. Coordinates/facilitates the development of treatment plans by collaborating with patients, families, and interdisciplinary team. Develops individualized plans based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs. Implements, reassesses, and adjusts treatment plans as necessary. Completes an interdisciplinary assessment tool with each patient and updates it each week. Partners with multi-disciplinary team in the treatment planning process, communicates frequently and effectively to ensure continuity and coordination of care. Documents thoroughly, timely, legibly and in compliance with regulatory requirements. Develops a clinically relevant transition/discharge plan in collaboration with patient, family, and interdisciplinary team. Conducts individual, group, couples, family therapy and psycho‑education groups. Demonstrates the ability to advance evidence‑based practice by utilizing standards, guidelines and pathways for care delivery and incorporating data to continually improve patient outcomes. Promotes a culture of evidence‑based practice through activities such as: Utilizing evidence‑based policies in clinical practice Attending and participating in learning opportunities about evidence‑based practice Membership in hospital or unit‑based practice/research councils Leading evidence‑based practice projects Conducting literature reviews Developing standards/pathways/guidelines for patient care Presentations and research studies Required Minimum Qualifications Master’s degree in psychology, social work, nursing or closely related field. Current Minnesota license in one of the following: Licensed Independent Clinical Social Worker (LICSW), Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), Licensed Alcohol and Drug Counselor (LADC). Basic Life Support certification prior to the end of orientation. Preferred Qualifications 2 years of supervised therapy experience in mental health/substance use disorders, including assessment, individual, group, and family therapy.
