Growing together.Position Highlights & Primary Responsibilities:Performs physical assessments on patients with specialized conditionsDevelop and implement a plan of care for these patients, as well as prescribe medications, exercises and/or other treatmentsCollaborates with the clinical and administrative teams in the management of patient care. Maintaining continuity of care through the exchange of information with outside health and social agenciesServes as a liaison between primary care providers and specialists such as general surgeons. Consults with primary care providers on issues related to patient careTeaches and counsels individuals and groups with regard to specific health care needs. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
