Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and comprehensive reviews of medical records to identify emotional, social, and environmental factors that are influencing patient recovery.

• Facilitate access to community resources, including support groups, vocational rehabilitation programs, and housing assistance to better equip patients post-rehabilitation.

• Offer educational support to patients and families regarding available community resources and aftercare services, ensuring they are well-informed and prepared for discharge.

• Participate actively in discharge planning, working to ensure smooth transitions from the detox center to home or alternate care settings.
