JOB RESPONSIBILITIES Patient Care - Age Specific and Culturally Inclusive- Consistently integrates age specific and culturally inclusive concepts into patient care, taking into consideration both the patient's chronological age and developmental functioning. Interdisciplinary Collaboration- Working for system improvement, promoting patient/family/caregiver well-being. Identify and advocate for the needs of the patient/families/caregivers within the healthcare system and community. Act as a liaison between the patient/family/caregiver within the medical team and outside agencies. Act as the mediator as needed with in the medical team on behalf of the patient/family/caregiver. Empower the patient/family/caregiver to problem-solve by exploring options of care when available and alternative plans, when necessary, to achieve desired outcomes. Facilitate communication and coordination between members of the health care team while involving the patient/family/caregiver in the decision-making process in order to minimize fragmentation of services. Psychosocial Assessment- Conduct a comprehensive psychosocial assessment of the patient/family's health care needs to identify the strengths, challenges and opportunities. Evaluate patient/family/ caregiver safety concerns. Intervene appropriately to reduce risk. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Utilize the psychosocial assessment to establish treatment intervention to be included in comprehensive plan of care. Psychosocial Interventions- Provide psychosocial services as identified in the patient's comprehensive plan of care. Provides supportive counseling with the goal of improving emotional coping and adherence to the treatment plan. Collaborate with referral sources and health care team members. Education of the patient/family/caregiver and members of the health care team about treatment options. Provide self-management support to high risk/complex patients/families to increase their skills and confidence to manage their chronic care conditions at home. Provide groups and programs for special populations as needed. Provide anticipatory guidance and planning to reduce or eliminate psychosocial barriers to discharge. Coordinate and facilitate family centered care team meetings. Provide crisis assessment and interventions to patients/families/caregivers with the goal of restoring equilibrium. Resource Development and Utilization- Develops resource networks, excels at resource utilization, and acts as a resource to peers. Identifies needs, develops programs, and evaluates outcomes for special populations, as needed, and implements changes based on outcomes. Facilitates collaboration with community agencies to enhance investigation and intervention process. Advocates for patient population on a systems level (hospital, organization, and/or community) by developing programs and protocols to better meet the needs of the patient population. Leadership- Serves as a recognized expert to health care team and/or community on social work practice. Recognized as an expert by the health care team resulting in an expanded role within the team and/or division. Provides work direction, leadership, mentoring and coaching to all personnel assigned to their location. Participants in recruitment and selection process for candidates that would work in their assigned location. Coordinates the work schedules for all clinical staff. Coordinates the clinical activities of the clinical staff. Meets with them routinely to meet the needs of the clinical staff and to assure continuity of care in all areas. Provides input for each employee's annual performance evaluation at their location. Participates in the development of long term and short term goals for their location as well as for each employee. Works as liaison between school districts and the division. Attends all monthly, quarterly meetings for school districts involving themselves and the staff they supervise. Compliance- Ensures that the clinical practice of all clinical staff conforms to local, state, and federal regulations and accrediting standards. Ensures clinical competency of all clinical staff. Responds to, documents, and analyzes complaints associated with therapy services. Maintains minimum level of continuing education required by State licensing board.
