In order to achieve our vision and deliver our mission, we need the best primary care providers and nurse practitioners that are seeking to fulfill purpose and personal opportunity and join the ChenMed family.We are seeking a mission-driven Nurse Practitioner to join our team in a part-time role, designed to strengthen care delivery across our centers. The primary focus of this role is to improve same-day access, support high-risk patient stabilization, reduce avoidable acute care utilization, and provide operational support during periods of provider capacity strain. This role is ideal for a clinician who thrives in a collaborative environment and is passionate about improving access, continuity and the overall patient experience.The Flex NP is intended to support high-impact clinical access and stabilization needs. Routine longitudinal panel management responsibilities remain with primary PCP team. Clinicians in this role are expected to exercise a high degree of clinical judgment and autonomy while collaborating closely with market leadership and care teams.As a Flex NP, you will serve as a trusted clinical partner across care teams helping ensure that every patient receives timely, high quality care. You will step into established care teams to support patient needs, maintain continuity of care and enhance the effectiveness of our primary care providers, without the responsibility of managing your own patient panel. You will be an essential part of delivering on our promise to provide more good days for both patients and care teams.This is a unique opportunity to practice medicine in a way that is highly relational, team oriented and focused on impact, while benefiting from a more flexible schedule. Primarily assigned to the West Philadelphia hub with periodic travel to regional centers as operational needs arise.How you'll make an impactDeliver comprehensive, patient-centered primary care aligned with our value-based care modelProvide seamless coverage across providers and centers, ensuring patients have consistent access to careSupport same day visits, follow ups, walk-ins, hospital discharges, careline escalations and acute care needs to maintain continuity for our patientsCollaborate closely with providers and care teams to reinforce individualized care plansBuild meaningful connections with patients that reinforce trust in their care teamContribute to a culture of love, accountability and passionWhat makes this role uniqueNo patient panel ownership – focus on care delivery without panel management responsibilitiesPurposeful flexibility – 3 day schedule including one weekend dayBroad clinical exposure – work across teams and patient populationsTeam design – Strengthen care delivery and provider effectiveness across the marketKnowledge, Skills and Abilities:Demonstrated record of consistently achieving clinical performance metrics in current roleStrong critical thinking and problem-solving skillsExcellent communication and interpersonal skillsTime management skills with the ability to work well under pressureMust be caring and empathetic and have great listening skillsMust be detail-oriented, and able to pay close attention to patient charts, medications, and follow-up on details of patient careBasic computer skills and some knowledge of Microsoft Office SuiteFlexible to work evening, weekends and/or holidays as neededPrimarily assigned to the West Philadelphia hub with periodic travel to regional centers as operational needs ariseEducation and Experience Criteria:ARNP or similar advanced degree in Nursing requiredCurrent Nurse Practitioner Certification in the State of practice requiredBoard certification by AANP or ANCC is preferred but may be required for certain StatesCurrent DEA number from the DEA for schedule II-V controlled substances may be required based on State of practiceMinimum 2 years of clinical experience as a Nurse Practitioner required practicing in Family Medicine, Internal Medicine, or Geriatrics, including past level of autonomy to make independent care decisionsBasic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employmentPay Range:$111,140 - $158,771 SalaryThe posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
