As a Social Worker at the Northern Metropolitan Center, you will have the opportunity to make a meaningful difference in the lives of our patients and residents by providing emotional, social, and psychological support.

About the Role:

As a Social Worker, you will work closely with our healthcare team to assess, develop, and implement individualized care plans that address the physical, emotional, and social needs of our patients and residents. Your responsibilities will include:

• Conducting social histories and assessments to identify patient/resident needs and develop personalized care plans
• Providing counseling, education, and support to patients, residents, and their families
• Coordinating with healthcare providers and other professionals to ensure comprehensive care
• Facilitating connections with community resources and services
• Maintaining accurate and comprehensive records of patient/resident care
• Participating in interdisciplinary team meetings and contributing to continuous quality improvement initiatives

Requirements:

• Minimum 0 years of experience in social work or a related field

• Bachelor's degree in Social Work or a related field

• Current licensure as a Social Worker (LSW, LCSW, etc.)

• Excellent communication, interpersonal, and problem-solving skills

• Ability to work effectively in a fast-paced environment with multiple priorities
