Contract | Monday - Friday, 8:00 AM - 5:00 PM | Onsite with Local Travel

Key Responsibilities

• Evaluate, develop, and implement individualized treatment plans to support participants in safely remaining in their homes and communities
• Conduct home-based safety assessments and provide skilled therapeutic interventions in the day center and therapy gym settings
• Help improve participant strength, mobility, daily living skills, and overall confidence
• Coordinate durable medical equipment (DME) and other supportive services to promote participant independence and quality of life
• Collaborate with an interdisciplinary team including physicians, nurses, therapists, and social workers to deliver comprehensive, coordinated care

• Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program
• Current NBCOT registration required
• Current Colorado Occupational Therapist license (or ability to obtain prior to start of employment)
• 2-3 years of Occupational Therapy experience
• Valid driver's license and ability to perform local travel between care settings

• Experience working with older adults or geriatric populations
• Familiarity with PACE (Program of All-Inclusive Care for the Elderly) or similar comprehensive senior care models
• Experience conducting home safety evaluations and DME coordination

• Work within a mission-driven, interdisciplinary care model focused on helping older adults maintain independence and quality of life in their own homes and communities
• Enjoy a consistent Monday-Friday daytime schedule with variety across multiple care settings
