Community Health Navigator & Outreach Advocate

La Casa de Salud in Bronx, NY seeks a Peer Navigator to support individuals facing barriers to care, including homelessness and behavioral health challenges. This field-based role uses lived experience to connect people with primary care, behavioral health, housing, and benefits services. You will document encounters, participate in care teams, and assist with outreach while maintaining HIPAA compliance.
