Job Description

This position is grant funded. Therefore, it is solely dependent of availability of grant funds. Provides healthcare and educational services for City residents by supporting vaccine administration, prenatal care, and other healthcare programs. Conducts home or telehealth visits, maintains patient documentation, and leads clerks and other medical staff operations.

Essential Functions/Job Competencies/Physical Requirements

Essential Duties and Responsibilities:

• Provides home and telehealth visits to first time mothers
• Utilizes motivational interviewing, develops care plans, refers clients to additional services, and monitors effectiveness of care plan
• Assesses client/child health, provides client education, coordinates care with other providers, and addresses social determinants of health
• Maintains accurate records and documentation of home and telehealth visits
• Participates in nursing and City training activities, as required
• Reviews nursing literature and evidence-based practices on maternal and child health
• Attends community coalition meetings, participates in public health emergency response, and attends outreach events
• Integrates all public health, maternal and child health, nutrition and allied health services to ensure good prenatal, postpartum, newborn, infant and well childcare during the first 2 years of life
• Conducts nursing home health services for maternal child health services (assures prenatal care compliance and newborn, well childcare, growth and development)
• Links all appropriate health care services from primary care, well child, Immunizations, WIC (Women, Infants and Children), social services with the department and with other agencies for appropriate prenatal, postpartum, newborn well child, and positive child growth and development
• Assists medical team in tracking and follow-up care for both mother and child
• Assures newborn has all well child services and provides health education and support for the first 2 years of life (includes observation between mother and child)
• Conducts quality assurance for patient care in the home setting
• Complies with a City of Laredo policies and procedures for nursing, public health services and administrative procedures. Will assist if necessary, in maternal child health services
• Will travel for additional training
• Conducts oneself in a professional and responsible manner
• Must follow rules of telephone courtesy in all telephone communications
• Will be required to drive a City vehicle for City business use
• Performs related work as required

Work Environment/Conditions and Physical Demands:

Physical Effort Requirements Position will have the following exposures:

• Occasional exposure to unpleasant environmental conditions and/or hazards. Occasional outside work.
• Exerting up to 35 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 20 pounds of force constantly having to move objects.

Work Environment Locations Position will require the employee to work in the following types of work environments:

• Never: Construction site, confined space, warehouse environment
• Sometimes: Street environment (near moving traffic)
• Frequently: Outdoor environment
• Often: Office or similar indoor environment, vehicles, client residence

Exposures Position will require the employee to be exposed to the following environmental elements:

• Never: Moving mechanical parts
• Seldom: Extreme cold (below 32 degrees)
• Sometimes: Individuals who are hostile or irate, individuals with known violent backgrounds, fumes or airborne particles, toxic or caustic chemicals or substances, loud noises (85+ decibels such as heavy trucks, jack hammers, construction)
• Frequently: Communicable diseases, animals, including pets
• Often: Extreme heat (above 100 degrees)

Travel Position will require the employee to travel:

• Never: International travel
• Sometimes: Regional travel, national travel
• Often: Local travel

Minimum Qualifications

Required Education and Experience:

• Associate's degree from an accredited* college or university or two (2) years of college coursework in Nursing or a related field
• At least one (1) year of experience in healthcare, vaccine administration, clinical experience in women's health, maternal-child, early childhood development or related experience

Proof of TB Test/Screening will be required to be presented prior to employment

Valid Licenses and Certifications

Required Licenses or Certifications:

• State of Texas Registered Nurse License (Must attach copy of your Nurse License and certification to your application.)
• CPR Certification
• Valid Texas Driver License. If applicant holds an out-of-state license, a State of Texas Driver License must be obtained within 6 months of employment.
