LCSW - Crisis Therapist

Top Behavioral Health group looking for a wonderful LCSW to join their Crisis Therapy team near Detroit, Michigan!

About:

-Crisis Therapists provide crisis intervention and case management for adults (18+) with serious mental illness or co-occurring substance use disorders who are at risk of psychiatric hospitalization, recently admitted to inpatient facilities, or prone to repeated emergency psychiatric services. They conduct comprehensive assessments to identify service needs, deescalate crises, and connect individuals to treatment and support services, coordinating with other providers and community resources.


-Therapists also offer telehealth screenings and crisis intervention, determining care levels and collaborating with local police. Services are available 24/7, with therapists sharing on-call responsibilities. Regular hours are Monday-Thursday, 8:30 am-7 pm, and Friday, 8:30 am-5 pm, with occasional evening or weekend shifts.

Responsibilities:

• Conduct screenings and assessments to identify client needs and service preferences.
• Provide clients and families with information on available services and resources.
• Assist in determining eligibility for community services and benefits.
• Coordinate with treatment teams and hospitals to support transitions to community-based care.
• Facilitate access to aftercare and provide transportation assistance if needed.
• Develop Individualized Plans of Service with clients and treatment teams.
• Provide in-person or telehealth services based on client needs.
• Conduct community outreach to engage or reengage clients.
• Offer crisis intervention and safety planning as needed.
• Collaborate with local providers and participate in team meetings.
• Document service activities promptly and maintain productivity standards.
• Keep appropriate licensure and complete required training.
• Follow organizational policies, code of ethics, and make necessary referrals.
• Perform other duties as assigned.
