Position Summary

As a Licensed Practical Nurse (LPN) , you will provide direct nursing care, administer medications, and support residents' overall health and well-being while working closely with the interdisciplinary care team.

Key Responsibilities

• Provide direct patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes to the clinical team
• Supervise and support CNAs to ensure high-quality care delivery
• Maintain accurate and timely clinical documentation
• Promote a safe, clean, and compassionate environment

Qualifications

• Active, unencumbered LPN license in South Carolina (required)
• Current CPR certification required
• Must maintain all required continuing education and remain in good standing with the State Board of Nursing
• Experience with PointClickCare (PCC) preferred
• Strong communication, teamwork, and clinical skills
• Compassion for an underserved population
