Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day shift, Monday - Friday Hours per Shift: 10 hours or 8 hours per day, dependent on operational needs Experience: Minimum of 2 years of case management experience. License: Valid Registered Nurse License Certifications: None specified Must-Have: 6 months of discharge planning case management experience in an acute care hospital, ideally in a large organization with 500 beds or more. Description: This position requires candidates to have at least 2 years of experience in case management. Recent experience in discharge planning case management is essential. The work schedule for this position may vary between four 10-hour shifts or five 8-hour shifts based on operational needs. Candidates should demonstrate knowledge of case management principles and practices. The ideal candidate will have experience working within a large teaching hospital environment.
