Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician directions. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for patients, ensuring the design of functional treatment programs aimed at allowing effective return to community activities. This role includes educating patients, families, students, coworkers, and self, while adhering to the organization's mission statement and core values, promoting compliance and confidentiality, and upholding professional standards.

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans focused on patient progress towards discharge.
• Maintain medical record documentation that is clear, concise, accurate, and timely.
• Ensure care is provided under valid physician orders and within insurance approval as necessary.
• Participate in student programs, providing oversight and a positive learning environment.
• Create individualized professional development plans and pursue ongoing professional growth.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Understand and follow organizational performance improvement methodologies.

• Degree in Physical Therapy from an accredited school (required).
• 1+ year of outpatient clinical experience (required).
• CPR Certification (required).
• Active and applicable state licensure (required).
• Previous assignments with Select Medical or related experience (highly desired).
