Wyckoff Heights Medical Center | Ambulatory Services 1610 Dekalb Ave Brooklyn, NY 11237, USA Wyckoff Heights Medical Center | Ambulatory Services 1610 Dekalb Ave Brooklyn, NY 11237, USA The Program Manager oversees the grant-funded Retention Adherence Program (RAP) and Undetectables Peer Learning Network (UND), coordinating the implementation of treatment and adherence services for assigned case management programs at Positive Health. This role supervises all Specialists and Peer Educators, provides direct service and case management to a small caseload of clients under the UND program, and ensures high-quality, coordinated care through staff training, case conferences, chart review, and ongoing quality improvement efforts. The Program Manager also serves as a key liaison between the program, community partners, and the Consumer Advisory Board. Coordinates the implementation of all components of the grant-funded RAP program Oversees the Undetectables Peer Learning Network (UND) grant-funded program Ensures the development of client-centered treatment plans and facilitates case conferences Provides financial incentives to clients for achievement of viral load suppression and maintains a financial incentives tracking log for the program Oversees implementation of treatment and adherence services for assigned case management programs and supervises all Specialists and Peer Educators Coordinates the weekly multi-disciplinary case conference and ensures active participation by all HIV RAP Case Management Teams Facilitates staff trainings in program-specific, evidence-based interventions and other quality service delivery strategies, ensuring appropriate implementation by program staff Trains staff in assessing patient needs appropriate to age, health conditions, and psychosocial factors, and in making relevant service referrals Implements and trains staff to deliver, with fidelity, evidence-based interventions required by the program or clinic, such as Motivational Interviewing and SBIRT Manages a small caseload of UND clients directly, providing assessments and reassessments, service care plan development, behavioral health counseling, crisis intervention, referrals, and coordination of services Ensures team coordination of follow-up on client referrals and appointments Provides backup to program staff as needed Serves as a community liaison for the Consumer Advisory Board (CAB), co-facilitating CAB meetings when requested and assisting with recruitment of members and related activities Develops and maintains active collaborations with community agencies and consumer venues to facilitate outreach, messaging/education, and referrals for clients served by the programs Oversees the development, updating, and procurement of promotional and educational materials for the RAP, UND, and BHX programs Facilitates integration of best practices and CQI initiatives across all HIV Case Management Programs Reviews charts for quality and completion and prepares a monthly report for review by the Director Monitors and reports program quality indicators during internal QI meetings and at provider meetings with the NYC DOHMH or other external partners Manages client tracking, supervises data collection, and ensures timely and accurate reporting and service delivery Oversees the implementation of new patient and annual behavioral health assessments as requested by medical providers in the clinic Participates in and leads Quality Assurance and Evaluation activities, trainings, supervision, and related activities Provides education and coaching to direct-service staff about identifying relational barriers to optimal patient engagement in care Participates in regular supervision with the Director of Supportive Services and attends required staff meetings Attends necessary trainings and in-services, including off-site meetings with the Department of Health and AIDS Institute partners, to remain current on best practices in HIV care and prevention Performs other job-related duties as assigned by the Director of Supportive Services and all other duties as required Position Requirements: Master's Degree (minimum formal education) Minimum of 3 years of job-related experience Ability to work independently Ability to prioritize and manage a caseload Ability to problem-solve and perform appropriate intervention strategies in acute crisis situations Ability to problem-solve outside routine area of coverage Positive and respectful interaction with patients, co-workers, providers, and outside stakeholders Desired (Not Required) Criteria: Master's Degree in Social Work or equivalent Health and Human Services field 3 years of job-related experience in HIV care, treatment, and behavioral health services LMSW or LCSW preferred 2-4 years of experience serving low-income communities that have experienced barriers to health care, stigma, and discrimination Experience delivering services in a patient-centered model of care Non - Union Monday - Friday / 9:00 AM - 5:00 PM Salary Range: $70,000 - $75,000 Qualifications Skills Behaviors Motivations Education Experience Licenses & Certifications Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.For further information, please review the Know Your Rights notice from the Department of Labor. #J-18808-Ljbffr
