Now Hiring: Physical Therapist (PT) – PRN Facility: Lake Eustis Healthcare & Rehabilitation Location: Eustis, Florida Schedule: PRN Pay Rate: $57 per hour About Us Lake Eustis Healthcare & Rehabilitation is a skilled nursing and rehabilitation facility committed to providing high-quality, resident-centered care in a compassionate and supportive environment. We are currently seeking a motivated PRN Physical Therapist to join our rehabilitation team and help residents reach their highest level of independence and mobility. Position Summary The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and delivering evidence-based therapy services to promote functional independence and overall well-being. Key Responsibilities Evaluate residents and establish appropriate treatment goals and therapy plans Implement physical therapy treatment programs and complete all required documentation accurately and timely Apply therapeutic techniques including gait training, ambulation, strengthening, coordination, endurance, mobility, posture control, balance, and sensory awareness Design, fit, and train residents in the use of adaptive devices, orthotics, and prosthetics Select and administer appropriate physical modalities based on resident needs Collaborate with the interdisciplinary rehabilitation team to ensure individualized and effective care plans Educate residents and families on mobility techniques and progress toward goals Modify therapy programs as residents’ conditions and needs evolve Participate in care plan meetings and interdisciplinary team conferences Supervise Physical Therapist Assistants (PTAs) and aides in accordance with state regulations Provide education and training to facility staff on physical therapy best practices Monitor therapy equipment and request supplies or maintenance as needed Promote a respectful, compassionate environment that supports residents’ dignity and rights Qualifications Current, active, and unencumbered Physical Therapist license in the state of Florida Minimum of 1 year of experience in a skilled nursing or long-term care setting preferred Strong clinical, communication, and documentation skills Ability to work collaboratively in a team-oriented environment If you are a passionate Physical Therapist seeking a flexible PRN opportunity in a supportive and resident-focused setting, we invite you to join our team at Lake Eustis Healthcare & Rehabilitation in Eustis, Florida.
