Referral Care Navigator
Neighborhood Healthcare
Neighborhood Healthcare in Riverside, California is seeking a Referrals Specialist to coordinate referrals and follow-up care for PACE participants. This role requires excellent communication and organizational skills and a minimum of one year of clinical experience. You will serve as the key contact for providers and will arrange transportation for participants to their medical appointments.
