Champion City Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist to join our full-time therapy team. This is an excellent opportunity to work in a collaborative skilled nursing environment where you can make a meaningful impact on residents’ mobility, safety, and independence.

The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled physical therapy services to improve strength, balance, mobility, and functional independence. The PT works closely with the interdisciplinary team to ensure optimal clinical outcomes and high-quality patient care.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care

• Provide skilled interventions including gait training, balance training, strengthening, transfer training, neuromuscular re-education, and functional mobility training

• Assess and recommend appropriate assistive devices and mobility equipment

Qualifications

• Graduate of an accredited Physical Therapy program

• Current Pennsylvania Physical Therapist license (required)

• Skilled nursing facility experiences preferred, but not required
